- This online service is available to
residents of Burbank, Illinois and persons living within the Nottingham
Park, Illinois.
- Those who live outside of Prairie
Trails Library District need to go to the library nearest their home
to apply for a library card.
- You will receive a temporary
library card number that expires in 14 days. If you are over 18 years
of age visit the library's Circulation Department and present two
forms of identification
verifying your name and Burbank or Nottingham Park address
within 14 days to complete the
registration process and receive a permanent library card number.
- If you are under 18 years of age
your legal guardian must present
two forms of identification verifying their name and Burbank or Nottingham
Park address at the library's Circulation Department to complete the
registration process and receive a permanent library card number for a
minor's library card.
- Accepted forms of ID:

(Either/Or,
both will not constitute two forms of identification)

Regulations
- Your library card may be used at
any SWAN Library. It is free to all residents of Burbank and Nottingham
Park, Illinois.
- You are responsible for all
materials borrowed on the card. It is also your responsibility to pay any
charges that may result from late return, lost or damaged items.
- Legal Guardians are responsible for all
materials borrowed on a minor's card. It is also the legal guardian's
responsibility to pay any charges that may result from late return, lost or
damaged items on said card.
- Treat your library card as you
would a credit card: report lost or stolen cards immediately to prevent
unauthorized use. Replacement cards:$3.00.
- The Library charges fines for
materials kept past the due date, and reserves the right to refuse further
service until the account is in good standing. The library employs a
third party to assist in retrieving fines on patron records.
- Please report changes of name or
address immediately.
- The information requested on this
application is held in strict confidence
by Prairie Trails Public Library District, and is used for library
purposes only.
I understand I am responsible for this card and all
materials checked out on this card until this card is reported lost or missing.
I am further responsible for any changes that may result from late return, loss,
or damage of materials borrowed. I agree to comply with the library and System
regulations.
