 BENEFITS OF LIBRARY CARD OWNERSHIP

 Residents living in the communities of Burbank and Nottingham Park are tax supporters of the Prairie Trails Public Library District. Our library offers popular fiction titles, large print books, audio-books, music CDs, DVD/videotapes, picture books, and more. Your Prairie Trails library card will be honored at any of the public libraries in the Metropolitan Library system. In addition, your library card entitles you to request circulating materials from libraries nationwide. These items will be delivered to the Prairie Trails Library location (State Road and Moody Avenue) for checkout. There will be no charge for materials requested and supplied by an Illinois library. There will be a fee charged for materials received from a library outside of the state of Illinois. Your library card will allow you remote access to online databases via the library’s website: www.praireitrailslibrary.org. The library also provides programming for youngsters, young adults, adults, and families. 

 

GETTING A LIBRARY CARD 

Adult residents (18 years of age or older):

Residents living in the communities of Burbank and Nottingham Park are tax supporters of the Prairie Trails Public Library District. Patrons must present two forms of identification proving residency in the Prairie Trails Public Library District. The only acceptable forms of identification are pre-printed with the applicant’s name and current address such as:

  • Utility Bill

  • Valid Driver’s License OR State I.D. (Either/or: both will not constitute two forms of identification)

  • Motor Vehicle Registration card

  • Property Transfer Certificate

  • Senior Citizen Membership Card

  • Voter Registration Card

  • School I.D.

  • Class Schedule

  • Lease or Mortgage Papers

  • Public Aid or Medical Aid I.D. Cards

  • Envelope with Postal “Change-of-Address” Label and Current Postmark

  • Current Magazine with Mailing Label (Printed)

  • Personnel Check – (Owner not receiver)

  • Any Preprinted Form or Bill with the Applicant’s Name and Current Address

 Juvenile Residents (17 years of age or younger)

Patrons must reside within the library district. Parents or legal guardians must present proof of residency as stated above and sign an application form assuming responsibility for materials loaned to juvenile patrons.

CARD RENEWAL

Your library card will be valid for a two-year period. To renew your library card, just bring it and one proof of residency from the above list of acceptable forms of identification to our circulation staff. If the expired card is not presented, there will be a $3 fee and a new card will be issued. All fines and fees on the account must be paid before a card may be renewed.

LOST OR STOLEN CARDS

When a card becomes lost or stolen, advise the library so that the card may be invalidated. When replacing your lost or stolen card bring one proof of residency from the above list of acceptable forms of identification to our circulation staff. You will be assessed a $3 replacement fee.

CHICAGO RESIDENTS

 You must present a valid Chicago Public Library card and one of the proper forms of identifications (listed previously) verifying your Chicago residence. The information on the Chicago Public Library card, the identification, and the application form must match. 

You will not receive your Metropolitan Library System reciprocal card on the day you apply for it. You will be contacted by the library when your card is available for pickup usually within three weeks. 

The Metropolitan Library System reciprocal card will expire in one year. To renew it your must complete an application card and present your reciprocal card, valid Chicago Library card, and proof of residency in Chicago, as previously stated. The Metropolitan Library System reciprocal card must be free of fees and fines at the time of renewal.
